Community Health Worker

A Community Health Worker is a frontline public health worker with a close understanding of the communities they serve. They are a link and intermediary between health care and social services, and the community to facilitate access to services and improve the quality of service delivery. A CHW helps to increase health knowledge and self-sufficiency through outreach, community education, informal counseling, social support and advocacy.
