Domestic and International Student Trip Documentation

Complete this form if you are planning a student trip and the trip meets the requirements found in the Trip Guidelines for Clery.

Student Trip Report

This form is required by the Clery Act as a way to track student overnight trips.
  • This should be the name of the employee responsible for the students on the trip.
  • This should be the person who made reservations or is completing this form on behalf of the Student Trip Sponsor.
  • Please indicate the department/division you represent: ie, Athletics, Agriculture, Upward Bound, MARET, Club or organization
  • Date Format: MM slash DD slash YYYY
    Please indicate the date you plan to leave campus
  • Date Format: MM slash DD slash YYYY
    Please indicate the date you plan to return to campus.
  • Please indicate the name of the hotel where you will be staying
  • Please indicate the room numbers (if available), floor locations, etc. for each hotel room occupied by our students
  • Please indicate any information you feel is pertinent to the trip.
