version of Adobe Acrobat Reader (5.0) to access printable form
To submit a request
in writing without using the Adobe form: type or print your request ,
be sure to include your name (if your name was different when you attended
- please indicated), social security number and the institution, including
address, where you would like the transcript sent.
Be sure to sign the request.

There is a $2 fee for transcripts being sent to other institutions - $5
if you wish it faxed. You can send the fee as a check, made payable to
Crowder College, to the address above - Records Office.
